This is my translation of the English language death record of Jan (John) Wierzba, from the St. Stanislaus Parish – Milwaukee, Wisconsin. Any errors are completely my own.

Milwaukee, 1915,

On the 4th of October, 1915, I, Rev. L. B. Jurasinski, Pastor of the St. Stanislaus Parish, officiated at the funeral of Jan Wierzba, aged 70. He died on the 30th of September, 1915. He was buried in the St. Adalbert’s cemetery.
