This is my translation of the Latin language baptismal record of Jan Wałdoch, from the Leśno Parish. Any errors are completely my own.

Trzebuń, 1801,

On March 23rd, 1801, I, Rev., Pastor of the Leśno Parish, baptized the infant named Jan. The parents were: Józef and Ewa Wałdoch nee Żywicka, the legally married couple. The Godparents were: Maciej Wałdoch and Helena Wałdoch, both from Trzebuń.
