This is my translation of the Russian language death record of Michał Tobiasz, from the Straszewo parish.
Zakrzewo No. 10 1905
This occurred in the village of Straszewo on the 17th of February/2nd March, 1905, at 9 a.m. Ignacy Maciejak, age 40, and Szczepan Jecerzembski?, age 30, both peasants from Zakrzewo, appeared and stated that on the 15th/28th of February, 1905, at 9 a.m., in Zakrzewo, Michał Tobiasz, an attendant, died, age 40. He was born in Biesiekierz, district of Koneck, resident of Zakrzewo, the son of Ignacy and Zofia Karpik. His widow is named Rozalia. After eyewitness testimony on the death of Michał Tobiasz, this document was read to those present, who were illiterate, and signed by us.
