This is my translation of the German language civil death record of Piotr Kleinsmyt (Kleinschmidt), from the Sierakowice Civil Registers. Any errors are completely my own.

Weronika Kleinschmidt has reported the death of Piotr Kleinschmidt. He resided in the village of Sierakowice Huta. His death was reported on November 7th, 1894.
