Death of Tomasz Kacprzak (1841)

This is my translation of the Polish language death record of Tomasz Kacprzak, from the Ostrowąż Parish. Any errors are completely my own.
Kijowiec, No. 9, 1841
This occurred in the village of Ostrowąż, on the 20th of April/2nd of May, 1841, at 2 p.m. Appearing are witnesses: Jan Mielcerak, aged 40, a laborer; and Florian Mielcerak, aged 35, both residing in the village of Kijowiec, and stated that in the village of Kijowiec, on the 18th/30th of April, 1841, at 10 a.m., Tomasz Kacprzak, aged 36, died. He was Catholic and a laborer residing in the village of Kijowiec. He was born in the village of Biele (should be Biechowo). His parents were the deceased Walenty and his widow Franciszka Kacprzak, who were laborers residing in the village of Kijowiec. He leaves behind his wife Marianna (should be Magdalena). After eyewitness testimony on the death of Tomasz Kacprzak, this document was read to those present, who were illiterate, and signed by Us.
