This is my translation of the Polish language death record of Katarzyna Gorzycka, from the Radziejów parish.
Kolonia Opatowice, No. 38, 1865
This occurred in the town of Radziejów, on the 22nd of September, 1865, at 2 p.m. Appearing are witnesses Antoni Banaszkiewicz, aged 45, and Andrzej Gardyza, aged 45, both are farm owners residing in Kolonia Opatowice, and stated that on the 20th of September, 1865, at 4 p.m., Katarzyna Gorzycka, aged 5 months, died. She was the daughter of Filip and Marianna Gorzycki nee Kocińska farm owners residing in Kolonia Opatowice. She was born and resided in Kolonia Opatowice. After eyewitness testimony on the death of Katarzyna Gorzycka, this document was read to those present, who were illiterate, and signed by us.
