This is my translation of the Polish language death record of Jadwiga Gorzycka, from the Radziejów parish.
Kolonia Opatowice, No. 40, 1852
This occurred in the town of Radziejów, on the 6th of July, 1852, at 12 p.m. Appearing are witnesses Antoni Banaszkiewicz, aged 35, the village mayor, and Walenty Karczmarek, aged 58, a farm owner, both residents of Kolonia Opatowice, and stated that on the 5th of July, 1852, at 1 p.m., Jadwiga Gorzycka, died. She was the daughter of Filip and Marianna Gorzycki, who are farm owners. After eyewitness testimony on the death of Jadwiga Gorzycka, this document was read to those present, who were illiterate, and signed by us.
