Baptism of Teresa Bielawska (1814)

This is my translation of the Polish language baptismal record of Teresa Bielawska, from the Jeziorsko Parish. Any errors are completely my own.

Jeziorsko, No. 32, 1814

On the 2nd of October, 1814, at 3 p.m., before Us, Rev. Franciszek Strzelińśki, the Administrator and Civil Registrar of Jeziorsko, performing the duties of the Registrar of Vital Records for the District of Jeziorsko, Warta County, Kalisz Province, Filip Bielawski appeared, aged 38, a chef residing in Wola Miłkowska, and presented Us a female child, which was born on the 30th of September, 1814, at 9 p.m., to his lawful wife Agata Pakuła, aged 24. At Holy Baptism, administered on this day, this child was given the name of Teresa. He made the above statement and showed Us the child, in the presence of witnesses: Jakób Badanowski?, aged 40, a blacksmith, and Mateusz Leniewicz, aged 43, an organist in Jeziorsko. After which this Birth Record was read to the declarant and witnesses, who are illiterate, and was signed by Us alone.
