This is my translation of the Polish language death record of Teofila Wawrzyniak, from the Straszewo Parish.
Zakrzewo, No. 21, 1859
This occurred in the village of Straszewo, on the 16th of April, 1859, at 5 p.m. Appearing are witnesses: Jan Wawrzyniak, aged 35, a farmhand, and Stanisław Kochanowski, aged 68, a laborer, both reside in Zakrzewo, and stated that in the village of Zakrzewo, on the 14th of April, 1859, at 2 p.m., Teofila Wawrzyniak, aged 2, died. She was the daughter of Jan and Łucja Wawrzyniak nee Kochanowska. After eyewitness testimony on the death of Teofila Wawrzyniak, this document was read to those present, who were illiterate, and signed by Us.
