Death of Kazimierz Żywiczka (1812)

This is my translation of the Polish language Death record of Kazimierz Żywiczka, from the Sędzin Parish. Any errors are completely my own.

Kuczkowo, 1812,

On the 30th of April, 1812, at 7 a.m., before Us, the Administrator and Civil Registrar of Sędzin, performing the duties of the Registrar of Vital Records for the District of Sędzin, Radziejów County, Mazowieckie Province, Wojciech Żywiczka, father of the deceased child, aged 40, a farm owner residing in Kuczkowo, and Krysztof Klein, a master blacksmith, aged 42, residing in Sędzin, and stated that on the 28th of April, 1812, at 8 p.m., in the village of Kuczkowo, in house #2, Kazimierz Żywiczka, aged 2, died. After which this Death Record was read to the declarant and witnesses, who are illiterate, and was signed by Us alone.
