This is my translation of the Polish language death record of Wojciech Pilachowski, from the Radziejów parish.

Miasto Radziejów, No. 188, 1852

This occurred in the town of Radziejów, on the 10th of September, 1852, at 8 a.m. Appearing are witnesses: Antoni Sałociński, aged 70, and Andrzej Wojciechowski, aged 64, both are laborers residing in Radziejów, stating that on the 9th of September, 1852, at 1 p.m., Wojciech Pilachowski, aged 50, died. He was a laborer residing in Radziejów. He leaves behind his widow Apolonia Pilachowska and his sons: Andrzej and Michał. After eyewitness testimony on the death of Wojciech Pilachowski, this document was read to those present, who were illiterate, and signed by Us.
