This is my translation of the Russian language death record of Antonina Tobiasz, from the Ostrowąs Parish.

Plebanka No. 6, 1896

This occurred in the settlement of Ostrowąs, on the 17th/29th of March, 1896, at 5 p.m. yesterday. Appearing are Michał So?, a servant, age 28, and Ignacy Matusziak, a servant, age 26, both residents of Plebanka, and stated that in Plebanka, on the 16th/28th of March, 1896, at 8 a.m., Antonina Tobiasz, age 2, died. Antonina was born to her parents Michał and Rozalia Tobiasz nee Nowak, in Zakrzewo. She died in the presence of her parents in Plebanka. After eyewitness testimony on the death of Antonina Tobiasz, this document was read to those present, who were illiterate, and signed by us.
