This is my translation of the Latin language baptismal record of Mateusz Wolęcki, from the Byczyna Parish. Any errors are completely my own.

On September 5th, 1812, I, Rev., Pastor of the Byczyna Parish, baptized the infant named Mateusz. The parents were: Stanisław and Teresa Wolęcki, the legally married couple. The Godparents were: Jan Spychał, from Nagórki, and Marianna Grzanka, from Ujma Mała.
