Death of Maciej Żywiczka (1819)

This is my translation of the Polish language Death record of Maciej Żywiczka, from the Sędzin Parish. Any errors are completely my own.

Kuczkowo, 1819, No. 9

On the 28th of May, 1819, at 12 p.m., before Us, the Administrator and Civil Registrar of Sędzin, performing the duties of the Registrar of Vital Records for the District of Sędzin, Radziejów County, Mazowieckie Province, Grzegorz Słoma and Wojciech Kempara, both are farm owners residing in Kuczkowo, and stated that on the 27th of May, 1819, at 4 p.m., in the village of Kuczkowo, in house #11, Maciej Żywiczka, aged 6 months, died. He was the son of Wojciech and Zofia Żywiczka. After which this Death Record was read to the declarant and witnesses, who are illiterate, and was signed by Us alone.
