Death of Andrzej Grzelak (1821)

This is my translation of the Polish language Death record of Andrzej Grzelak, from the Radziejów Parish. Any errors are completely my own.

Opatowice, 1821, No. 37,

On the 17th of November, 1821, before Us, the Administrator and Civil Registrar of Radziejów, performing the duties of the Registrar of Vital Records for the District of Radziejów, Radziejów County, Mazowieckie Province, Jakób Grzelak appeared, aged 40, a farm owner from Opatowice, the son of the deceased; along with Antoni Brzeziński, aged 30, a farm owner residing in Opatowice, and stated that on the 16th of November, 1821, at 12 a.m., Andrzej Grzelak, aged 90, died in house #11. He was a laborer residing in Opatowice. He leaves behind his widow Marianna nee Król, aged 60. After which this Death Record was read to the declarant and witnesses, who are illiterate, and was signed by Us alone.
