This is my translation of the Latin language baptismal record of Zofia Pilachowska, from the Radziejów Parish. Any errors are completely my own.

Biskupice, 1809, No. 37,

On May 14th, 1809, I, Rev. Leopold Wnorowski, Pastor of the Radziejów Parish, baptized the infant named Zofia. Her parents were Jan and Marianna Pisiak, the legally married couple. The Godparents were: Ferdinand Mogiel? and Ewa Jędrzejewski, both from Radziejów.
