This is my translation of the Latin language baptismal record of Teodor Grzelak, from the Radziejów Parish. Any errors are completely my own.

Opatowice, 1801, No. 74,

On November 22nd, 1801, I, Rev. Norbert Zawichowski, Pastor of the Radziejów Parish, baptized the infant named Teodor. His parents were Andrzej, the village mayor, and Marianna Grzelak, the legally married couple. The Godparents were: Mateusz Jędrzejewski and Franciszka Zdrojewska.
