This is my translation of the Polish language death record of Feliks Kiciński, from the Sompolno Parish.

Sompolno, No. 10, 1845

This occurred in the city of Sompolno, on the 22nd of April, 1845, at 9 a.m. Appearing are witnesses: Leonard Domiński, aged 62, and Andrzej Michalski, aged 32, both residing in Sompolno, and stated that on the 20th of April, 1845, at 8 p.m., in Sompolno, Feliks Kiciński, aged 6, died. He was the son of Jan and Wiktoria Kiciński nee Muszyńska. After eyewitness testimony on the death of Feliks Kiciński, this document was read to those present, who were illiterate, and signed by Us.
