This is my translation of the Polish language death record of Jakób Grzelak, from the Radziejów parish.

Opatowice, No. 30, 1847

This occurred in the town of Radziejów, on the 15th of April, 1847, at 3 p.m. Appearing are witnesses Józef Banaszkiewicz, aged 63, a farm owner, and Teodor Gralik, aged 48, both residing in the village of Opatowice, stating that on the 12th of May, 1847, at 7 p.m., Jakób Grzelak, aged 63, died. He was a laborer residing in the village of Opatowice. He leaves behind his son Paweł and daughters Marianna and Antonina. After eyewitness testimony on the death of Jakób Grzelak, this document was read to those present, who were illiterate, and signed by Us.
