This is my translation of the Polish language baptismal record of Ełżbieta Bielawska, from the Chełmno Parish. Any errors are completely my own.
Chełmno, 1809
This is the legal statement of the birth of a child made by the father.
On the 29th of October, 1809, at 3 p.m., before Us, the Administrator and Civil Registrar of Chełmno, performing the duties of the Registrar of Vital Records for the District of Chełmno, Łęczyca County, Warszawa Province, Filip Bielawski appeared, aged 27, a master chef in the service of and residing on the Chełmno estate, and presented Us a female child, which was born in house #1, on the 28th of October, 1809, at 8 a.m., to his lawful wife Agata Pakuła, aged 21. At Holy Baptism, administered on this day, this child was given the name of Ełżbieta. He made the above statement and showed Us the child, in the presence of witnesses: Jan Horst, aged 21, an actuary residing in Chełmno, and Franciszek Adamczewski, aged 40, a master gardener residing in Chełmno. After which this Birth Record was read to the declarant and witnesses, Jan Horst is literate and Franciszek Adamczewski is illiterate, and was signed by Us alone.
