This is my translation of the Polish language death record of Józef Jędrzejczak, from the Krzywosądz Parish.

Czołpin, No. 27, 1830

This occurred in the village of Krzywosądz, on the 24th of May, 1830, at 12 p.m. Appearing are witnesses: Mateusz Szperka, aged 30, and Aleksy Pakoła, aged 67, farm owners residing in the village of Czołpin, and stated that in the village of Czołpin, on the 22nd of May, 1830, at 8 p.m., Józef Jędrzejczak, aged 5, died. He was the son of Szymon and Marianna Jędrzejczak (her surname was listed as Lewandowska), who reside in the village of Czołpin. After eyewitness testimony on the death of Józef Jędrzejczak, this document was read to those present, who were illiterate, and signed by Us.
