This is my translation of the Latin language baptismal record of Teresa Nowak, from the Chlewiska Parish.

On September 13, 1807, I, Rev. Antoni Rojewski, Pastor of the Chlewiska Parish, baptized the infant named Teresa. She was born on the 7th of September, 1807. Her parents were Antoni and Agnieszka Nowak, the legally married couple. The Godparents were Krysztof Kmieciak and Horonata Kwiatkowska.
