This is my translation of the Polish language death record of Maciej (listed as Mateusz on baptismal record) Jędrzejczak, from the Siniarzewo Parish.
Gosławice, No. 22, 1851
This occurred in the village of Siniarzewo, on the 14th of November, 1851, at 10 a.m. Appearing are witnesses Antoni Dąbek, aged 40, an estate shepherd, and Tomasz Błachowicz, aged 30, a farmer, both residents of Gosławice, and stated that on the 11th of November, 1851, at 6 p.m., Mateusz Jędrzejczak, aged 2, died. He was born in Gosławice, the son of Paweł and Teresa Jędrzejczak, who are both laborers. After eyewitness testimony on the death of Mateusz Jędrzejczak, this document was read to those present, who were illiterate, and signed by us.
