Baptism of Szczepan Tobiasz (1912)

This is my translation of the Russian language baptismal record of Szczepan Tobiasz, from the Straszewo parish. Any errors are completely my own.
Zakrzewo No. 82 1912
This occurred in the village of Straszewo on the 11th/24rd of November, 1912, at 3? p.m. Jozef Tobiasz appeared, age 35, a laborer, resident of Zakrzewo, in the presence of Michał Lewandowski, age 37, and Jan Kos??, age 42, both residents of Zakrzewo, and presented us a male child, stating that the child was born in the village of Zakrzewo, on November 7th/20th, 1912, at 3 p.m., to his lawful wife Katarzyna Brzezińska, age 31. At Holy Baptism, administered on this day, by Rev. Wacław Linowiecki, this child was given the name of Szczepan. The Godparents were: Władysław Brzeziński? and Józefa Wojciechowska. This document was read to the declarant and witnesses, who are illiterate, and was signed by us alone.
