This is my translation of the Russian language death record of Anna Urbańska nee Jędrzejczak, from the Zgłowiączka Parish. Any errors are completely my own.
Milżynek, No. 22, 1903
This occurred in the village of Zgłowiączka, on the 28th of April/11th of May, 1903, at 10 a.m. Appearing are witnesses: Paweł Przybylski, aged 33, and Ludwik Kolaczkowski, aged 45, both residing in the village of Milżynek, and stated that on the 27th of April/10th of May, 1903, at 10 a.m., in the village of Milżynek, Anna Urbańska, aged 33, died. She was born in the village of Gosławice, Gmina Krzywosądz, Powiat Nieszawa. She was the daughter of Paweł and Teresa Jędrzejczak nee Skonieczna. She leaves behind her husband Kazimierz Urbański, who resides in the village of Milżynek. After eyewitness testimony on the death of Anna Urbańska, this document was read to those present, who were illiterate, and signed by Us.
