Baptism of Helena Deręgowska (1809)

This is my translation of the Polish language baptismal record of Helena Deręgowska, from the Piotrków Parish. Any errors are completely my own.

Świesz, 1809

On the 5th of March, 1809, before Us, the Administrator and Civil Registrar of Piotrków, performing the duties of the Registrar of Vital Records for the District of Piotrków, Radziejów County, Mazowieckie Province, Franciszek Deręgowski appeared, aged 50, a farmer, and presented Us a female child, which was born in house #1, on the 5th of March, 1809, to his lawful wife Apolonia nee Krzysztofiak, aged 40. At Holy Baptism, administered on this day, this child was given the name of Helena Franciszka. He made the above statement and showed Us the child, in the presence of witnesses: ????. After which this Birth Record was read to the declarant and witnesses, who are illiterate, and was signed by Us alone.
