This is my interpretation of the Canadian civil death record of Anthony (Antoni) Yakubek (Jakubek), from the Renfrew County Death Registrar. Any errors are completely my own.
Horton Township, April 15, 1914,
Anthony Yakubek, aged 83, married, a laborer, residing in Horton Township, Renfrew County, Ontario, Canada, died on April 4th, 1914. He was born in Germany (Poland) and his parents names are unknown. His primary cause of death was due to senility, with a secondary cause associated to asthma.
