This is my translation of the Latin language baptismal record of Jakub Jakubek, from the Lipusz Parish. Any errors are completely my own.

Tuszkowy, 1820,

On July 17th, 1820, I, Rev., Pastor of the Lipusz Parish, baptized the infant named Jakub. The parents were: Szymon and Regina Jakubek nee Wierzba, the legally married couple. The child was born on the 16th of July, 1820. The Godparents were: Bartłomiej Wierzba and Rozalia Wierzba.
